Ileal transposition is a metabolic or bariatric procedure that is used in the treatment of diabetic patients. This procedure is more commonly used for patients who are obese or overweight. What happens in an ileal transposition?

The beginning of the surgical procedure includes a sleeve gastrectomy. A sleeve gastrectomy is a surgical procedure that focuses on weight loss procedures. In the procedure, a part of the stomach, about 80%, is removed from the person’s body. This removal is done along the greater curvature of the stomach. After the process is completed, one can get an ileal transposition. There are two methods of getting an ileal transposition.

Diverted (Duodeno-ileal interposition): In this procedure, once sleeve gastrectomy is completed, the connection between the stomach and the duodenum is closed. Then a part of the ileum about 170 cm is cut off and then connected to the first part of the duodenum. The part of the duodenum is at the end of the stomach. The other end of the ileum is then connected to the proximal portion of the intestine. Hence, after the procedure is complete, the ileum is interposed between the stomach and the proximal part of the intestine. The duodenum and the proximal portion of the small intestine are not usable anymore; hence the patient is required to go through bypass surgery. After this procedure is completed, the patient will have lesser bodyweight and controlled blood sugar. But they will also have a chance of getting anemic, an iron deficiency in the body. This will be because of the bypass surgery.

Non-Diverted (Jejuno-ileal interposition): In this procedure, sleeve gastrectomy is done, and then a part of the ileum about 200 cm long is cut off. This part is then interposed to the proximal portion of the small intestine. Since the stomach is left unbothered in this procedure, the food continues to pass through the intestine. There is no malabsorption as the duodenum absorbs food normally. The hormones released by the duodenum play a significant role in controlling and changing blood sugar. Hence, the weight is maintained in this procedure, but the blood sugar is not managed as effectively as the diverted procedure.

Who qualifies for an ileal transposition?

An ileal transposition is done to control the blood sugar and weight of a person. This will be recommended to a patient by the doctor or the surgeon when the person is obese or overweight and suffers from type 2 diabetes.
