Pyeloplasty

Pyeloplasty is the surgery performed to treat the medical condition called Utero-pelvic Junction Obstruction. This involves removing a blockage that obstructs the urine to reach the bladder. “Pyelo” refers to the human kidney and “Plasty” refers to the use of repairing or restoring in the surgical procedure.

Why is Pyeloplasty Performed?

A pyeloplasty is done when the tube draining the urine from the kidney into the urinary bladder gets blocked. This forces the urine to be pushed back into the kidney. This causes the loss of kidney function, pain, or infection. This area is called the ureteropelvic junction.

In most cases, the blockage of the tubes before birth is diagnosed when a swollen kidney results in an ultrasound. After the birth, imaging tests are done to find the cause of the surgery and whether surgery is needed to unblock the tubes.

In other cases, symptoms start showing up indicating the blockage of the tubes. The symptoms include the following:

  • Urinary tract infections
  • Vomiting
  • Severe pain in the abdomen
  • Blood in the urine
  • Kidney stones

How Pyeloplasty is Performed?

Pyeloplasty surgery is carried out in three possible ways:

  • Open Pyeloplasty: In this, the skin and tissues are removed making incisions on the skin. This allows the surgeon to see the under the skin. This is considered safe in infants or babies.
  • Laparoscopic pyeloplasty: In this, the skin and tissues are removed using an instrument called a laparoscope. The laparoscope is attached with a camera and the light at the end. Small incisions are made to pass this instrument into the skin. This surgery is recommended for adults with UPJ obstruction.
  • Robotics pyeloplasty: In this, the surgeon uses the computer to control the movement of the robotic arm under the skin.

Before the surgery:

A specific time is assigned by your doctor during which you are not allowed to eat or drink anything. General anaesthesia is given to put you to sleep to avoid any discomfort or pain. The catheter is put into place and left overnight.

During the Surgery:

  • During an Open pyeloplasty, incisions of two to three inches are made under the ribs. The obstructed ureter is then removed. A normal calibre ureter is attached to the kidney. A tiny silicone tube called a stent is placed to drain the urine from the kidney. After the recovery from pyeloplasty, the stent is removed. This surgery is performed in babies or infants as it is considered safer than laparoscopy.
  • During a Laparoscopic or robotic pyeloplasty, multiple small inches ranging between 8 to 10 millimetres are made. The laparoscope is then inserted to cut the narrow tissue, thereby fixing the obstruction. However, in robotic pyeloplasty, the robot assisting the surgeon has three to four robotic arms. One arm holds the camera and the rest are attached with the instruments. These instruments move similarly to a human hand. These fix the obstruction by removing the scarred tissue and reconnecting the normal tissue. This surgery is done in adults or older children. The surgery lasts for three hours.

What Are the Side Effects of Pyeloplasty?

Pyeloplasty includes the following side effects or risks:

  • Infection
  • Swelling
  • Bleeding
  • During the surgery, the urine may get drained in other areas and cause infections or irritation
  • The tube may get blocked again
  • Injury to the majority of the blood vessels
  • Injury to the different organs

Who Are the Right Candidates for Pyeloplasty?

The best candidates for the surgery of pyeloplasty include:

  • Infants whose condition does not improve within 18 months
  • Older children, teenagers, or adults with UPJ obstruction or kidney obstruction

How effective is pyeloplasty?

A pyeloplasty is considered to be 85% to 100% effective of all time.

What would happen without the pyeloplasty?

If the pyeloplasty is not performed, the urine remains trapped. This leads to forming of kidney stones or kidney infections resulting in kidney dysfunction.

What are the alternatives considered opposed to pyeloplasty?

Balloon dilation: this involves using a balloon to stretch the narrowed area that is passed up from the bladder. This does not involve any incisions; however, it is not recommended for all cases.
