Ileal Transposition Surgery in C-scheme, Jaipur

Ileal transposition is performed to treat overweight diabetic patients. This technique was developed in 1999 by Brazilian surgeon Aureo De Paula. The ileum is the distal part of the small intestine. It is responsible for further digestion of the food coming from the stomach. It absorbs the nutrients and the water so that it can be used by the body. The nearest part of the small intestine is the duodenum. It is responsible for the breakdown of the food. The jejunum is the third part of the small intestine that is located between the ileum and the duodenum.

Ileal Transposition is the surgical translocation of the ileum between the stomach and the duodenum or by placing the ileum to the duodenum. This surgery involves reducing the size of the stomach by sleeve gastrectomy. Sleeve Gastrectomy is the surgery for weight loss.

What Is the Procedure for Ileal Transposition?

A patient is given general anaesthesia and is placed in the supine position as the surgery involves operating in the lower part of the abdomen. Initially, sleeve gastrectomy is performed in a standard manner. In non-obese patients, a loose sleeve gastrectomy is performed to restrict the food intake and adjust the BMI. There are two techniques for the transposition at Apollo Spectra, Jaipur:

  • Diverted interposition: From the second level of the duodenum, the connection between the stomach and duodenum is closed. The 170cm segment of the ileum is created and then connected to the first segment of the duodenum. This preserves the last 30 cm of the small intestine. The other end of the ileum is connected to the proximal part of the small intestine. Thus, the ileum is interposed between the stomach and the duodenum. This is also called Duodeno-ileal transposition.
  • Non-diverted interposition: In this technique, the 200 cm segment of the ileum is created. It is then interposed to the proximal part of the small intestine. During this, the 30 cm part of the small intestine is preserved. This is also called Jejuno-ileal transposition.
